Nationwide Laboratory Services’ Continuous Quality Improvement Protocol organizes the CQI process to achieve three primary goals:
- Demonstrates and documents Network outcomes for state and network auditors
- Identifies individual patients not obtaining clinic’s goals and or crossing a clinical threshold requiring intervention i.e. a prescription change
- Provides a "full proof system" to ensure short term care plans are updated to include prescription changes based on the clinics protocols
Network Disease State Management Reports:
- Calculates, and graphs the % of patients achieving network goals by month
- Graphing facilitates assessing the success of your present protocols, benchmarking your outcomes relative to others and enables you to track and assess your clinic’s progress towards achieving network goals
- Tool organizes and documents CQI process for State and Network Auditors et al
Custom CQI Reports:
- Identifies individual patients not obtaining your clinics goals
- Calculates the percentage of patients not obtaining clinic’s goals
- Isolates patients that require a prescription change - based on your clinics protocols
Custom Short-Term Care Plans:
- Provides a short term care plan to document prescription changes
- Care plan includes the last four monthly blood draws (20 tests) for each patient
- Documents each multidisciplinary team member’s "sign off" on their individual
intervention(s)
